Your rights
- To be treated with dignity and respect, and to receive care without discrimination.
- To be told, in plain language, what care is planned, who will provide it, and what to expect.
- To take part in decisions about your care, and to refuse treatment after being told what that may mean.
- To privacy for your health information, and to see and get a copy of your record.
- To know the name and role of the people caring for you.
- To be told about your options for care — including what is available in your area — and to choose freely.
- To have your questions answered and to be told what your options are if a service is not covered.
- To be free from abuse, neglect and exploitation, and to report it if you experience or witness it.
- To voice a complaint without being punished, and to be told how to complain.
Your responsibilities
- Give us accurate information about your health, medicines and coverage that we need to care for you safely.
- Follow the care plan you agreed to with your physician and nurse, and tell us when you cannot.
- Keep the visits you have agreed to, or tell us as early as you can if something changes.
- Treat our nurses and office team with courtesy, and keep the visit environment safe.
- Tell us right away if a wound gets worse or you do not feel well.
Complaints
